Blood Establishment Annual Activity Report

All blood establishments must maintain records in relation to the prescribed activities for which they are responsible. This is a requirement of Article 9 paragraph 4 of S.I. 360 of 2005.

This information should be submitted in a report to the HPRA on an annual basis. Blood establishments must submit the completed and signed form to the HPRA by the 1st of March.
